LMSW Case Manager, Social Work

Kaiser Permanente is seeking an LMSW Case Manager to support in-patient social work services. In this role, you’ll manage complex patient cases, coordinate care plans, and connect patients and families with community and clinical resources. You’ll collaborate closely with interdisciplinary medical teams to support safe discharges, address psychosocial needs, and advocate for patient rights and preferences. This position requires strong assessment, documentation, and crisis intervention skills, along with a deep commitment to patient-centered care in a fast-paced hospital environment.
