Community Care Physicians seeks an Outpatient Nurse Practitioner or Physician Assistant to deliver patient-centered care in our multispecialty, physician-owned group in Latham, NY. In this role, you will manage acute and chronic conditions, perform exams, order and interpret tests, prescribe medications, and provide preventive and wellness-focused care.
