RN Acute Care Transition Specialties

UCHealth seeks an RN Acute Care Transition Specialties nurse to support safe, patient-centered transitions from hospital to home or post-acute settings. In this role, you will assess patients’ clinical and psychosocial needs, develop individualized transition plans, coordinate with physicians and interdisciplinary teams, and provide clear discharge teaching and follow-up instructions. You will use evidence-based protocols, EHR tools, and community resources to reduce readmissions, improve outcomes, and support UCHealth’s culture of innovation, teamwork, and continuous learning.
