Speech Language Pathologist

CenterWell Home Health seeks a PRN Speech Language Pathologist to provide in-home evaluation and treatment for seniors and adults with chronic conditions. Responsibilities include assessing speech, language, cognitive-communication, and swallowing disorders; developing and implementing individualized care plans; educating patients and caregivers; documenting in EHR; and collaborating with an interdisciplinary team. You’ll support safe independence at home while working within a compassionate, patient-first culture that values teamwork, professional growth, and high-quality, personalized care.
