You assess medical, psychosocial, and discharge needs, develop individualized care plans, and collaborate with physicians, nursing, and Social Services. Using tools such as InterQual, you monitor progression of care, address barriers, and communicate with patients, families, and external providers to support quality outcomes and appropriate post-acute services.
