Physician Assistant – In-Home Health Assessments

Signify Health is seeking a Physician Assistant to perform comprehensive in-home clinical assessments for diverse patient populations. You will review medical histories, perform physical exams, reconcile medications, and document findings in EMR systems to support value-based care programs. Partnering with interdisciplinary teams, you’ll develop care recommendations, close care gaps, and coordinate follow-up with primary and specialty providers.
