Medical Social Worker

Lehigh Valley Health Network is seeking a Medical Social Worker to support patients and families across our regional health system. In this role, you will complete psychosocial assessments, coordinate discharge plans, connect patients with community resources, and provide counseling and crisis intervention. You will collaborate closely with physicians, nurses, and care teams to remove barriers to care, support complex medical and social needs, and promote safe transitions.
