Job Summary
The LPN is responsible for providing exceptional nursing care to our patients at the practices. Within the scope of practice, assumes responsibility, as necessary, for the assessment, treatment and care of patients. Displays an understanding of the developmental needs of the patient when planning and implementing treatment. Displays competency in obtaining information, familiarity with medications, and processing of refill prescriptions. Works closely with the Physician and other members of the clinical team. May perform phlebotomy and/or injections for the patient and make patient callbacks with lab test results and diagnosis. Responsible for providing documented information in the patients chart accurately within the EMR system.
Essential Functions
- Systematically provides nursing care to patients within the framework of the nursing process and established standards of practice.
- 2. Integrates principles of patient safety in the provision of direct nursing care and medication management.
- 3. Adheres to established system-wide communication standards and processes.
- 4. Participates in designated forums which address patient care, patient education, policy and procedures, safety and clinical outcomes.
- 5. Demonstrates accountability for maintaining and improving professional knowledge and skill related to the area of practice.
- 6. Other duties as assigned.
