Community Care Physicians seeks an Outpatient Nurse Practitioner or Physician Assistant to provide patient-centered primary care in our multispecialty medical group in Latham, NY. In this role, you will evaluate and manage patients with acute and chronic conditions, order and interpret diagnostics, prescribe medications, and deliver education that supports long-term wellness.
