Case Manager, Social Work

Kaiser Permanente seeks a Case Manager, Social Work – In Patient II to support in-patient members through comprehensive psychosocial assessment, care planning, and coordinated discharge. In this hybrid role, you’ll collaborate with physicians, nurses, and behavioral health teams to develop and implement patient-centered treatment and transition plans, ensure continuity of care, and connect patients and families with community resources. You’ll document in EHR systems, support crisis intervention, uphold regulatory and privacy standards, and contribute to continuous improvement in care delivery and outcomes.
